Employee's fingertip amputated in dryer conveying system

On January 4, 1994, Employee #1 was inspecting the Niro dryer conveying system for moisture. Forgetting that the sifter airlock was on, e removed the Venturi manifold to dry it out. Employee #1 then placed his left index finger in the airlock Venturi hole to check for moisture, amputating the tip. Signed training records indicated that he had received proper training. A "court of review" determined that the accident was avoidable and that Employee #1 had been in violation of work rule #20. He was issued a written warning.
