Investigation Summary
At approximately 9:22 a.m. on June 10, 1999, Employee #1, a structural steel welder employed by Knight Steel, Inc., was attempting to reposition a 8,200 lb beam at the steel fabrication facility. The beam shifted and fell over onto him. Employee #1 sustained massive internal injuries and was transported by paramedics to Arrowhead Regional Medical Center, where he died.
