At approximately 10:48 a.m. on February 22, 1999, Employee #1, of Auto Extras, was helping to coat segments of carpeting for eventual use in the manufacture of automotive floor mats when his right index finger was amputated. He was transported to Loma Linda University Medical Center, where he was treated and released.
