Employee Amputates Fingers on Unguarded Cutter Head

At approximately 10:30 a.m. on March 17, 1999, Employee #1, a woodworking machine operator, was running test material through the cutter head of a SCMI 30 wood shaper machine, Model Number T 130N/AB37121, without the cutter head guard in place. He amputated his right thumb, index and middle fingers. He was hospitalized for treatment.
