Employee injured while operating press

On January 30, 2003, Employee #1, working for company that manufactured electronic cables and harnesses for the aerospace and other industries, was working in the molding department placing metal connector pins in molds to be heated between hot plates (clamps) of a press. Normally two buttons are required to be pushed to activate the press. However, at the time of the accident, the two-handed control was not operating. The left button was either broken or tampered with so that two hands were not required to activate the ram. The employee was cleaning the ram with grease prior to placing epoxy material into the press and contacted the right button with his body. The ram came down and caught his glove and left thumb. The employee sustained a distal tip amputation of his left thumb.
