Investigation Summary
On July 19, 2005, Employee #1 was helping to remove excess bottles from inside the machine under the conveyor line. The machine is Number 8, OMSO Novax M, Screen Printing Machine. The operator of the machine turned it off at the controls on the front of the machine before asking Employee #1 to assist in removing the bottles. The machine operator was also removing bottles from under the conveyor, and was located at the opposite end of the machine. The operator apparently finished removing her bottles and then turned the machine back on without first checking to see if Employee #1 had finished removing the bottles at her end. The machine has interlocks on the sliding doors, which were inoperative, which allowed the machine to start up with the doors open. Employee #1's right arm was still in the machine and her forearm and hand were crushed in the moving parts of the conveyor. Employee #1's forearm was fractured and the index and third fingers have nerve damage. Employee #1 was hospitalized.
