Investigation Summary
At approximately 10:00 a.m. on January 19, 2006, Employee #1, was performing maintenance on a Resina Screw Capper, Serial Number RC 3775473, Model Number 040 589. This machine is used to tighten the tops on shampoo bottles. According to Employee #1, bottles on the conveyor belt inside the machine were falling over. Without locking out the machine, he reached into the opening of the output side with his right arm to straighten the bottles. The small finger of his gloved hand caught in the hard rubber chain of the machine, was crushed, and amputated. Employee #1 was not hospitalized. A Plexiglas guard extended 19 inches from the point of operation.
