Investigation Summary
On December 14, 2003, Employee #1, a machine operator, was assisting a forklift operator to pick up a scale by holding the lifting handle on the scale as the forks were inserted into the handle. Employee #1's hand and finger became pinched between the fork and the handle. He was hospitalized at Kaweah Delta Hospital for a finger tip amputation.
