Investigation Summary
On December 9, 2004, Employee #1 was standing on a 12-foot mobile ladder stand to retrieve a box of merchandise from a storage rack in a warehouse. He positioned the ladder near the storage racks, where the box was located, but was unable to place it parallel and directly adjacent to the racks due to a pallet of metal pieces that was located on the floor directly in front of the rack. He then climbed 10 feet, 3 inches to the level where the merchandise was stored and leaned over to reach the box. As he grabbed the box and placed it on his right shoulder, Employee #1 lost his balance and grabbed onto the metal storage rack as he fell to the floor. His pinky finger on his right hand got caught on the corner of the rack, resulting in its amputation. Employee #1 was transported to Health-line Medical Group by the facilities operations manager and was later transferred to the California Hand Center. Due to nerve and tissue damage to the finger, it was able to be reattached.
