Investigation Summary
At approximately 7:50 p.m. on June 9, 2003, Employee #1, a support person at a residential home which serves the mentally disabled adults, was trying to have a client make her lunch for an activity the next day. The client became upset, refused, and ran down a hallway with Employee #1 following. A struggle ensued and Employee #1 fell, striking her head against a wall in the hallway. Later that night, after complaining of confusion and memory loss, Employee #1 was transported by family members to an emergency room, and subsequently hospitalized for treatment of the head injury. The supervisor was not at the home when the incident occurred.
