Investigation Summary
An employee was sitting atop a fuel tank, using a 610-millimeter-long pipe wrench to install a 102-millimeter-diameter riser. He was tightening the riser to the tank when the wrench slipped out of his hands. This caused the employee to lose his balance and fall approximately 2.7 meters to the ground. He sustained a serious injury to his lower back and was transported to a hospital, where he was treated for 5 days.
