Investigation Summary
At approximately 10:45 a.m. on November 5, 2007, an employee wanted to bring some commercial fan units from the lower parking level to the ground level by a rental forklift. While bringing the forklift to lower level, the employee recognized the ceiling height getting lower and almost touch the forklift mass. The employee stopped the forklift and asked other foreman assisting him to pull forklift out by using company truck. The other foreman instructed a helper driving company truck to where the forklift stuck. After cable connecting completion, the employee planed getting back and putting forklift in reverse to help the pull. Within seconds after connecting cable, the employee still had not stepped out the way; the truck driver stepped hard on gas pedal, gear still on reverse without receiving clear signal from the employee and other spot man. The truck hit the forklift at its left corner where the employee was pinned. The employee was taken to Santa Monica UCLA then transferred to Westwood UCLA Medical Center and was hospitalized for two weeks. As a result of the accident, the employee fractured his right leg and pelvis.
