Investigation Summary
At 9:30 a.m. on February 2, 2004, Employee #1 was operating a 10-in., right tilt, Delta table saw. While reaching for a piece of cut wood, his right fingers contacted the unguarded blade. Employee #1 sustained the amputation of his right ring and little fingers to the second knuckle. There were no witnesses to the accident.
