Employee is injured while operating motion master machine

At 12:00 a.m. on September 20, 2003, Employee #1 and four coworkers were working the night shift at an audio manufacturing facility. Employee #1 was operating a CNC motion master machine, Serial Number 00830-0034, Model Number MTS 3004-42, in the shop area. As the computerized machine made its programmed cuts, Employee #1 stood in front of the machine, approximately 3 to 4 feet away, and picked up the parts. During the shift, a tool pit fell from a tool holder located at the rear of the machine. While the machine was running, Employee #1 walked to the back of the machine to retrieve the fallen tool. As he leaned down to pick up the tool, he rested his left hand on the spiral head of the machine. The unit moved and his hand was crushed between the unit and the spiral head. Employee #1 was transported to Holly Cross Medical Center and treated for his injury.
