Investigation Summary
On February 20, 2003, Employee #1 was working in the warehouse when he was asked by a coworker to help him unload some metal racks from a delivery truck at the loading dock. Employee #1 went to the loading dock and started to unload the racks along with two coworkers. After they finished unloading the metal racks, Employee #1 indicated he was standing on the dock behind the delivery truck with one foot on the bumper and one foot on the rubber stop of the loading dock. The roll-up door on the delivery truck was open and the truck moved. Employee #1 does not know how he fell and does not remember anything until waking up in the hospital. The store manager indicated that Employee #1 was standing on the loading dock near the trash bins. The store manager indicated that he observed the truck driver close the roll-up door, lock it, and walk towards the front of the truck. The store manager then heard the truck start up, and he started to walk toward the ramp to exit the loading dock. As the manager walked on the ramp he saw the truck pull away with Employee #1 standing on the ledge of the truck with his hands up against the door. The manager then saw Employee #1 jump off the truck, which was about a 4 ft drop. Employee #1 landed on his back and did not move. Employee #1 was hospitalized with a blunt head trauma, right frontal contusion to forehead with an abrasion, and a frontal lobe cortical bleed.
