Investigation Summary
On November 18, 2004, Employee #1 had removed and replaced a V-belt on the exhaust motor located on the roof. Employee #1 had completed the belt replacement with the power source deenergized. Employee #1 asked someone in the kitchen to turn the switch for the exhaust motor "on", while he was on the roof, so he could check if the belt was working properly. Employee #1 then replaced the cover on the motor where the V- belt was located. Employee #1's right hand came in contact with the moving V-belt and pulley. Employee #1 suffered the amputation of a portion of the little finger and damage to his ring finger. Employee #1 was hospitalized. Employee #1's amputated finger could not be reattached.
