Investigation Summary
Employee #1 was working for his employer, a restaurant supply warehouse company in Van Nuys, California. A coworker, a forklift operator at the same warehouse, was working in a refrigerated area restricted to employees-only, moving pallets of deli stock on the floor for final placement into stock. Employee #1 was standing outside the refrigerated area. While operating his forklift the coworker noticed a customer enter the employee-only area. Employee #2 was navigating his forklift through the area to avoid the customer when he hit Employee #1 outside the refrigerated area pinning Employee #1 against a wall. Employee #1 was transported to Northridge Hospital, Northridge, California. Employee #1 was admitted to hospital and stayed for three and one half days for observation before being released.
