Investigation Summary
On January 12, 2002, Employee #1 was painting an exterior wall in a parking lot. The employee was using a spray gun while standing on an extension ladder. Employee #1 stated that he was almost finished at the top of the wall when the base of his ladder slipped back and he fell. The wall was 13 ft 6 in. high. The ladder was a Werner Type 1A, Extra Heavy Duty with a 300 lbs capacity, Model number D1540-2, total length of 40 ft and maximum working length of 35 ft. The ladder did not have any visible defects. Employee #1 states that a coworker was painting a low wall with his back to the ladder. In addition, Employee #1 stated that the ladder was on a flat asphalt surface and that a sheet of plastic was on the ground next to the feet of the ladder. A coworker stated he was preparing some paint at the time of the accident and that the ladder was on an incline and that the feet of the ladder were placed on top of a piece of plastic. The surface measured to have approximately one-degree of incline. The ladder was not secured by being blocked, tied or secured by other means. Employee #1 fell and was hospitalized with a broken hip bone, broken left wrist, broken nose and cuts on his left knee.
