Employee Dies When Pinned Between Van and Workbench

On November 7, 2005, Employee #1, a mechanic, was standing in front of the rack and hoist and was guiding a van being loaded onto a rack. The van dropped off since its brakes were not functioning properly. Owner of the vehicle spoke to the shop foreman regarding the problem. The van was parked in front of the garage bay. One of two mechanics got into the van to drive it onto the rack. The front end of the rack had wheel stops. The back end had ramps that enabled the wheels to easily go on the top of the rack. Driver of the van drove the vehicle to the rack ramps and applied the gas pedal to give it enough horsepower to get it on the top of the rack. When the van got on the rack level, its motion was too fast. When the driver applied the brakes, the brakes failed. The van jumped the wheel stops and pinned Employee #1 between the van and the workbench. Employee #1 sustained multiple fractures and serious abdominal injuries. He died about five hours later.
