Investigation Summary
Employee #1 was performing maintenance and repair work on the "Mezzanine" level of a building. The establishment has tenants that edit motion picture films for showing movie theaters. There are stairs that allow access to the projection room and the projection room has a vertical hatchway that leads to the mezzanine on the other side of the wall. A step ladder is used to enter the hatchway. The passageway has areas large enough to allow standing and other areas where employees must crawl on their knees or duck under the duct work to gain access. The mezzanine floor is composed of sheets of plywood. There are cable-ways throughout the floor that employees must walk on or crawl over to gain access to some areas. On one particular area of the mezzanine has a break in the plywood floor that is octagonal-shaped. There is an approximately 2-in. drop in this area. In this area, what appears to be a floor is actually ceiling tiles for the first floor. Employee #1 stepped on the tile and fell 16 ft. to the floor. The floor consisted of hard industrial-type carpet (similar to indoor/outdoor) on top of concrete. Employee #1 suffered a dislocated shoulder and fractured elbow joint. Employee #1 was hospitalized.
