Investigation Summary
At approximately 5:54 a.m. on November 1, 2003, an employee, a truck driver, was conducting an inspection of a truck as required by the Department of Transportation. The inspection is required by DOT before a truck can be driven out of the yard. The employee said that he had the hood of the truck up with the engine running, looking for leaks, inspecting the fan belt, and doing a standard inspection. He said he was standing on the right side of the truck engine. According to the employee, his right hand got too close to the fan belt and got caught in the belt. The hand was whipped up over the top of the alternator. At this point he was able to pull his hand out. His right index finger was broken in two places with the skin ripped open. The middle finger was dislocated with compound fracture at the middle knuckle. The employee was hospitalized and treated for cuts and laceration.
