Investigation Summary
On February 25, 2002, Employee #1, a concrete forms worker, needed a short piece of wood to reinforce the forms he was making and he used a portable Skillsaw with the guard wedged in place to prevent the guard from operating automatically. Employee #1 was holding the work piece in his left hand when a knot in the wood pulled the piece from his hand, pulling his thumb into the path of the saw blade which severed the thumb from his hand. Employee #1 was hospitalized.
