Employee Died Falling from a Ladder

Employee #1 died when he fell 20 to 24 feet from an extension ladder while putting on an ornamental chimney cover on the chimney stack. The height of the chimney stack measured approximately 24 feet. Employee #1 used a 12-foot aluminum extension ladder. The ladder measured 24 feet long at the time of inspection. Based on the position where the ladder landed, the feet of the ladder were positioned 3 to 5 feet away from the vertical sides of the chimney stack, which resulted in an acute pitch angle. Employee #1 failed to secure, block, or tie the ladder to prevent displacement. Employee #1 fell 20 to 24 feet to his death.
