Investigation Summary
At approximately 11:00 a.m. on September 9, 2001, Employee #1, of CHP Steel, and a coworker of the general contractor, SME Construction, Inc., were working on the RCC Channel Project. The coworker, a crane operator, was hoisting a spreader bar with a steel mat attached, that was being guided by Employee #1 for placement on a wooden channel structure under construction. Employee #1 was using a wire to guide the mat when the spreader bar hoisting equipment contacted 16-kilovolt power lines. Employee #1 received an electric shock, suffering third-degree burns to his right hand, his right and left legs, and his stomach. The employer was cited for an accident-related violation, serious, of T8CCR 2946(b)(3), concerning provisions for preventing accidents due to proximity to overhead lines.
