Investigation Summary
An employee was operating the "Crown" stand-up forklift, Model RR5020-35, Serial Number 1A204139, in the "receiving area" of the warehouse. The employee operating the forklift had been instructed by another employee to go and pick up a pallet of material from the warehouse in Aisle R22B, and take it to a designated area. The employee drove the forklift into the aisle where Employee #1 was standing. The forklift operator drove past Employee #1 and stopped in front of Rack 22B where the material was stored. As the forklift approached the rack, the operator turned the forklift steering knob to the right, causing the back of the forklift to swing to the left and strike Employee #1 standing next to the rack. Employee #1 was pinned between the rack and the back of the forklift and received serious injuries to his chest. Employee #1 was hospitalized for seven days and had surgery to "drain blood from his lungs."
