Investigation Summary
Two employees at a hydroelectric generating plant were replacing control cables for two compressors. One of the employees used a 7.3-meter-long extension ladder manufactured by Bauer Corporation to access a cable tray located about 4.3 meters above the floor. As he was securing the ladder with hemp rope, the rope broke. The employee had been pulling the end of the rope to form a knot, and he fell about 3 meters to the concrete floor when the rope broke. He landed on his feet. He was hospitalized for 12 days with fractures of both ankles and his right wrist.
