Investigation Summary
On July 29, 2000, Employee #1 was laying decking for a steel structure when he fell approximately 23 ft from the leading edge onto the concrete floor. Employee #1 sustained a fractured left heel, a fractured pelvis, and a severe strain to his left arm. Causal factors of this accident include Employee #1 not being adequately trained on company safety policies, and the failure of the employer to utilize any type of fall protection for employees performing leading edge work.
