Employee falls in elevator shaft and dies

Employee #1 was assisting in placing plastic sheeting over window openings at a high rise hotel under construction. He and a foreman from the general contractor were the only persons on site. The foreman left him on the outside scaffolding and went to the roof. Approximately 30 minutes later the foreman found Employee #1 at the bottom of an elevator shaft. Employee #1 suffered multiple traumas and died.
