Employee's arm crushed in mill cutting machine

At approximately 3:40 p.m. on November 3, 1999, Employee #1 was sharpening the blades of a mill cutting machine that had ceased to cut the material properly. He was at the back of the machine, sharpening one of the blades, when his right hand became caught and was pulled between two revolving rolls up to his forearm. Employee #1 managed to stop the machine by relieving the pneumatic valve and, with the help of a coworker, free his arm. He was transported by paramedics to St. Francis Medical Center, where he was hospitalized with crushing injuries to his arm. The machine was not stopped, locked out, or mechanically blocked out. The employer was cited for an accident-related violation, serious, of T8CCR 3314(a). Employee #1 was a machine operator in the processing department of a company engaged in distribution of upholstery textiles,
