Employee's leg fractured in fall from fixed ladder

At approximately 3:30 a.m. on October 13, 1998, Employee #1, a crane operator, was preparing to start work with the #2 Transtainer crane in the container yard. He was wearing regular safety shoes and gloves, and had a backpack on his shoulder. Employee #1 was ascending a fixed ladder on the crane when, as he reached the cage portion of the upper ladder, his gloved hands slipped from the ladder rung. He fell approximately 20 ft through the 7 ft cage and then in front of the lower ladder to the ground. Employee #1 sustained serious multiple fractures of his right leg and a back injury. The fixed steel ladder was in compliance with regulations and no accident-related violation were found. Employee #1 later stated that the round steel ladder rungs were wet due to the early morning dew.
