Investigation Summary
At approximately 7:15 a.m. on November 12, 1998, Employee #1 was working on the generator of hydrogen generation unit #1 at the Texaco refinery in Wilmington, CA. He was trying to determine the cause of inadequate condensate flow to the sight glass of vessel #947 during start up of generator A. Employee #1 couldn't observe the level in the sight glass, so he walked the condensate line to find the line valve. When he approached the edge of the platform and still had not located the valve, he stood up on the middle rail of the handrail that protected the edge of the walkway from platform #1 to platform #2. Employee #1 was leaning over the top handrail to see if he could spot the valve when he lost his balance and fell approximately 21 ft onto the concrete floor. Coworkers in the immediate area heard Employee #1 fall and rushed to his assistance. Paramedics stabilized him and transported him to St. Mary's Hospital, where he underwent surgery on the orbit of his right eye.
