Investigation Summary
At approximately 3:54 p.m. on April 9, 1998, Employee #1, of the Cerritos (CA) Home Depot, was using a Raymond loadhog stand-up forklift to clean up and organize pallets on racks at the rear of the store. As he swung his vehicle counterclockwise from the open gate, his left heel, which was protruding from the truck, became pinned against the post gate support. Employee #1 sustained a crushed and fractured left heel, for which he was hospitalized.
