Investigation Summary
At approximately 8:30 a.m. on July 7, 1997, Employee #1, a quality inspector, was assisting in production on the express pie line machine. She picked up the pie tins with her left hand and placed them on the conveyor with her right hand. As she did this, the little finger on her right hand became caught between a sprocket and a sprocket chain drive in motion, and was amputated. Employee #1 was treated at the Western Hand Center. The accident occurred because the sprocket/sprocket chain drives were unguarded.
