Investigation Summary
Employee #1, who had been an hourly employee for a wholesale nursery and greenhouse operation since February 1995, was moving lumber on a steep incline using a Massey Ferguson industrial tractor with ROPS, model 40B, equipped with a front end loader bucket. He fell from the tractor and was hospitalized with severe lacerations to his upper right leg near the groin and to his lower right ankle. Employee #1 was not wearing a seat belt; the seat cushion was not appropriate for the vehicle; and the seat cushion was not secured to the frame.
