Mechanic fractures hand between guard and belt sheave

Employee #1, a maintenance mechanic who had been with the employer for six years, was inside a trim saw's housing replacing a broken v-belt. The trim saw's pneumatic piston which raises and lowers the saw blade descended, breaking bones in the employee's right hand. Employee #1 forgot to complete the lockout procedure; specifically, he did not deactivate the air pressure and bleed the reservoir to prevent movement.
