Employee's finger severed by saw blade

Employee #1, who had been employed by this workplace for a year, was cleaning a hula saw, a machine that has two blades activated by a control arm at hip level. He did not use the safety lockout and the little finger on his right hand was severed while he was standing at the operator's station.
