Employee injures arm when caught by automatic sander

Employee #1, the leadperson in charge of the swing shift, was sanding small parts using an automatic power sander (a Time Saver speedbelt sander). He was making an adjustment to the machine while it was still running. Apparently, Employee #1 did not use the appropriate de-energizing procedures when making the adjustments to the machine. Consequently, he was caught under the feed rolls of the automatic sander. Employee #1 suffered severe nerve and tendon damage to his right arm in addition to a fracture of the collateral ligament bone in his elbow.
