Employee crushed between pipes

Employee #1 and coworkers were removing pipes from a stack for inspection and routing to cleaning or further processing. The employees inspected the pipes for oil on the O.D. and checked code number symbols on each pipe's ID. One employee worked alongside the pipes on a forklift, checking ID symbols. A second employee was operating a forklift. A third employee was on the stack of pipes, out of the line of sight of Employee #1. Employee #1 was between a forklift with pipes, and the pipe stack, checking the O.D. for oil spots. The forklift was a 15/30 ton vehicle, carrying 40-foot lengths of 24 inch diameter pipe with 1/2-inch thick walls. The pipes were stacked three high. Employee #1 was crushed between two pipes and received four broken ribs, as well as damage to his pulmonary veins that required open heart surgery. He also sustained a bruised lung and subclavian artery damage. The pipes were not adequately chocked or secured.
