Investigation Summary
At approximately 2:00 p.m. on February 24, 2000, Employee #1 was part of a crew engaged in structural steel erection of a 2-story city college building. The crew had used a crane to install the I-beams and the steel posts approximately 2 weeks prior. Employee #1 was removing safety posts along the grid line on the roof's perimeter to accommodate the installation of wood framing on the roof. Employee #1 climbed onto a rolling scaffold at the second floor then walked on a large I-beam, then moved toward the middle-south end of the steel structure to remove some safety posts and wooden boards. Employee #1 was on a smaller I-beam bending down to remove a wood splinter on his left leg when the I-beam began to wobble from side to side. Employee #1 lost his balance and fall. Employee #1 attempted to grab the smaller I-beam on his way down, but all that did was to cause him to flip around and to fall face down on the second floor. There were no witnesses to the accident. Employee #1 was trained and has 16 years experience. Employee #1 sustained a multiple fractures in his left leg, ankles, wrist, jaw and a nose laceration as result of the 15-ft fall onto the metal decking below. Employee #1 was hospitalized.Note: The Injury Line of this report lists a fatality but the narrative states the employee was hospitalized for two days.
