Investigation Summary
At approximately 9:55 a.m. on March 17, 1999, Employee #1, a building maintenance engineer, was obtaining technical specs for air filters and belts for an air conditioning unit mounted on the top of an office space. There was no guard railing around the perimeter of the top of the office. Employee #1 fell approximately 10 ft while walking toward the ladder used for access to the top of the office space. Employee #1's left leg struck the ladder and she landed face first on the concrete floor below. Employee #1 was flown to a local trauma center. Employee #1's injuries were fractured facial bones, pelvis and wrist. Employee #1 was hospitalized.
