Maintenance employee's thumb amputated in machine

At approximately 6:00 a.m. on July 2, 1998, Employee #1, a maintenance mechanic, attempted to place a chain over a sprocket. He went under the machine to check the problem without first checking to see if the power to the machine was turned off. While attempting to replace the chain, which had come off the sprocket drive, the machine started and his right thumb was completely amputated. He was hospitalized and his thumb was reattached. He told his supervisor that he had not followed the company lockout policy. He was trained for the task which he was performing and the company had a safety program.
