Investigation Summary
At approximately 5:00 p.m. on July 30, 1994, Employee #1 was painting the top part of a 32 ft high wall of a house. He was standing on a ladder that was 36 ft long. The ladder was not tied off or secured to prevent slipping. As Employee #1 extended his arm to reach the top of the wall, the ladder slipped and he fell with the ladder. Employee #1 sustained a broken upper right leg and a head injury. He was hospitalized. After the investigation, the employer was cited for a serious violation of T8CCR 1675(j). Other violations were found and the employer was cited accordingly.
