Investigation Summary
At approximately 7.45 p.m. on March 14, 2001, Employee #1 was injured after he fell from a ladder into a stairway while working on the second floor of storage area at the back of a retail store. Employee #1 fell approximately 9 ft and sprained his back, neck, legs and lumbar. Employee #1 was standing on a 4-ft V-shape portable metal ladder, attempting to remove a box from the top shelf of a storage rack on the second floor when a few boxes fell off the shelf onto him. Employee #1 lost balance and fell off the ladder into an adjacent stairway. Employee #1 also stated that he was standing on the second step down from the top of the ladder and was working alone in storage area when accident occurred. Employee #1 was hospitalized.
