Employee amputates finger in press

On August 17, 1999, Employee #1 was operating the Roller Former Number 3. After removing the last piece of material from a roll of metal, he placed his right hand on top of the die body. Then he pushed the channel forward causing the die to hit the whisker switch. This action initiated the press causing it to descend and crush his right ring finger between the upper press and the die body. The standard operating procedure (SOP) for the equipment was to turn off the machine and disconnect the air hose prior to the extraction of the last piece. Further, Employee #1 did not disengage all three required switches to secure the power to the machine before removing the last piece. These failures to comply with the SOP for the equipment contributed to the amputation of his finger. He was transported to the hospital where he underwent surgery to repair the damage caused by the accident.
