Employee's finger amputated in table saw

At approximately 7:20 a.m. on April 8, 1999, Employee #1, of the Southcoast Cabinet Company, was using a table saw to cut a piece of wood in the assembly room. His left index finger struck the rotating blade and was amputated. There were no witnesses to the accident and Employee #1 did not return calls or respond to a mailed questionnaire concerning the incident. According to the employer, Employee #1 had not lowered the guard over the blade before beginning his cutting operations.
