Investigation Summary
At approximately 9:55 p.m. on April 15, 1999, Employee #1 and two coworkers were in the sculptor's shop. According to the employer, they were moving a 20 lb A-shaped piece of metal when it pivoted, fell to one side, and struck Employee #1 on the head. He was knocked to the floor and sustained injuries that required hospitalization. Employee #1 stated that he was working on a sculpture in the shop while a coworker was manually moving the metal frame nearby. The frame pivoted and fell, striking him on the head and knocking him down.
