Investigation Summary
At approximately 2:30 p.m. on January 13, 2000, Employee #1 was on a 6 ft metal and plastic ladder, bucking rivets on the inside walls and panels of a 48 ft trailer. The ladder tipped over and he fell between 4 and 5 ft onto the trailer bed, fracturing several ribs and puncturing his right lung. Employee #1 was transported to San Dimas Community Hospital, where he was treated for six days. A coworker was outside the trailer shooting rivets, but he did not see Employee #1 fall.
