Investigation Summary
At approximately 7:40 a.m. on September 21, 1999, Employee #1, of Andrews and Associates, and two coworkers were installing lamps under the ceiling of the cold storage room at Driftwood Dairy in El Monte, CA. Employee #1 was on a platform elevated on the forks of a lift truck when he and the platform fell between 6 and 9 ft to the floor. He suffered a fractured rib on his left side. The platform was not secured to the forks of the lift truck.
