Investigation Summary
At approximately 6:30 a.m. on October 17, 1997, Employee #1, of Royal Custom Design in Covina, CA, was using a Houdaille Powermatic table saw, model #69, serial #468028, to cut an approximately 18 in. long by 4 1/2 in. wide by 2 in. tall piece of wood to make a table leg when the board kicked back. It struck his left eye, breaking his prescription glasses and resulting in the loss of his eye. The employer was cited for serious violations of T8CCR 4300(d) and T8CCR 3382(a).
