Employee's fingertip amputated in gear assembly
Employee #1 was at his work station, a built-in tray-making machine with a belt/pulley assembly on one end that had been constructed in house. His left index finger became caught in the moving gears, amputating the tip. The belt/pulley assembly was not guarded.
